Accident Summary: ON 1L APRIL 1969 ^CWO NESBITT, CHARLES H.,^ AN INSTRUCTOR PILOT FOR THE ^57TH AVIATION COMPANY (AML),^ WAS GIVING ^WO MAXWELL, MAURICE D,^ HIS AIRCRAFT COMMANDER CHECKRIDE IN A UH-1H, SER NO 66-16534. ^CWO NESBITT AND WO MAXWELL^ PERFORMED THE PREFLIGHT INSPECTION ON THE AIRCRAFT. THIS NECESSITATED LEAVING THE OTHER CARGO DOOR OPEN ALSO. AFTER CRANKING THE PILOTS PERFORMED FIVE HOVERING AUTOROTIONS AND THEN ENTERED TRAFFIC AT KONTUM ARMY AIRFIELD TO PRACTIVE STRAIGHT-IN AUTOROTATIONS. ^CWO NESBITT^ DEMONSTRATED TWO AUTOROTATIONS AND THEN INSTRUCTED ^WO MAXWELL^ TO PERFORM THE NEXT AUTOROTATION. ^WO MAXWELL^ ENTERED AUTOROTATION OVER THE THRESHOLD O FRUNWAY 09. APPROXIMATELY TEN SECONDS AFTER ENTERING AUTOROTATION A FIELD JACKET BLEW OUT OF THE CE COMPARTMENT AND BECAME ENTANGLED IN THE TAIL ROTOR. THE FIELD JACKET CAUSED THE TAIL ROTOR BLADES TO SEPARATE FROM THE 90 DEGREE GEARBOX TO SEPARATE FROM THE TAIL BOOM. UPON DETERMINING THAT THERE WAS A TAIL ROTOR FAILURE ^CWO NESBITT^ TOOK CONTROL OF THE AIRCRAFT AND CONTINUED THE AUTOROTATION. AT APPROXIMATELY 100 FEET AGL ^CWO NESBITT^ INITIATED A SLIGHT DECELERATION TO DETERMINE THE ACTION OF THE AIRCRAFT. THE DECELERATION CAUSED THE NOSE OF THE AIRCRAFT TO TURN FARTHER TO THE RIGHT THAN THE TEN DEGREES RIGHT OF CENTER ATTITUDE WHICH THE AIRCRAFT HAD TAKEN UPON LOSS OF THE TAIL ROTORS AND GEARBOX. WHEN HE DETERMINED THAT IT WOULD BE IMPOSSIBLE TO DISSIPATE AIRSPEED WITH A CYCLIC FLARE AND STILL MAINTAIN DIRECTIONAL CONTROL OF THE AIRCRAFT, ^CWO NESBITT^ INITIATED A CYCLIC TURN TO THE RIGHT IN ORDER TO LOOSE AIRSPEED AND REMAIN WITHIN THE CG LIMITS IMPOSED UPON HIM BY THE LOSS OF THE 90 DEGREE GEARBOX AND TAIL ROTORS. WHILE EXECUTING THE TURN ^CWO NESBITT^ CONTROLLED THE RATE OF TURN AND DESCENT WITH THE COLLECTICOLLECTIVE. ^CWO NESBITT^ CONTINUED THE TURN TO A HEADING OF APPROXIMATELY 270 DEGREES. THIS PLACED THE AIRCRAFT APPROXIMATELY 75 TO 100 FEET SOUTH OF THE RUNWAY, 10 TO 15 FEET AGL AND 180 DEGREES FROM HIS HEADING UPON ENTRY INTO THE AUTOROTATION. AT THIS TIME ^CWO NESBITT^ ASSUMED A LANDING ATTITUDE AND APPLIED COLLECTIVE PITCH TO COMPLETE THE AUTOROTATION. THE APPLICATION OF AFT CYCLIC AND COLLECTIVE PITCH CAUSED THE AIRCRAFT TO TURN 90 DEGEES TO THE RIGHT BEFORE MAKING GROUND CONTACT. THE AIRCRAFT MADE GROUND CONTACT HEADING APPROXIMATELY 360 DEGREES. THE LEFT REAR SKID MADE GROUND CONTACT FIRST. THE LEFT SKID FOLDED UNDER DUE TO EXCESSIVE SIDE LOADING UPON GROUND CONTACT. THE RIGHT SKID WAS BENT OUT TO THE RIGHT UPON GROUND CONTACT. THE AIRCRAFT DID NOT BOUNCE UPON MAKING GROUND CONTACT. ^CWO NESBITT^ INSTRUCTED ^WO MAXWELL^ TO DEPART THE AIRCRAFT WHILE HE WAS SHUTTING THE AIRCRAFT DOWN. THE SITUATION WHICH CAUSED THIS ACCIDENT WAS CREATED BY AN IMPROPER PREFLIGHT BY THE PILOTS. AN UNSECURED FIELD JACKET IN THE CE COMPARTMENT WAS OVERLOOKED DURING THE PREFLIGHT. THE FIELD JACKET BLEW OUT OF THE AIRCRAFT AND BECAME ENTANGLED IN THE TAIL ROTOR CAUSING THE 90 DEGREE GEARBOX AND THE TAIL ROTOR BLADES TO SEPARATE FROM THE AIRCRAFT. THE TAIL BOOM SUSTAINED STRESS CRACKS DUE TO THE SUDDEN STOPPAGE OF THE TAIL ROTOR. BOTH SKIDS, BOTH CROSSTUBES AND THE CROSSTUBE HARD POINTS WERE DAMAGED DUE TO EXCESS SIDE LOADING ON THE SKIDS UPON GROUND CONTACT.
