Accident Summary:
UH-1H AIRCRAFT NUMBER ^67-17248^, UNDER THE COMMAND OF ^WO1 TERRY C.
LEE,^
HAD ENTERED A PICK-UP ZONE AS CHALK 2 OF A FLIGHT OF 5 ARICRAFT. AFTER PICKING
UP 5 PASSENGERS, ^MR.
LEE^ DEPARTED THE PZ AND CLIMBED TO AN ALTITUDE OF 5000
FEET AS INDICATED ON THE ALTIMETER WHICH WAS APPROXIMATELY 3000 FEET ABOVE THE
GROUND. AFTER REACHING THIS ALTITUDE, ^MR.
LEE^ ENTERED A LEFT HAND ORBIT WHILE
WAITING FOR ALL SHIPS OF THE FLIGHT TO JOIN UP BEFORE DEPARTING FOR THE LANDING
ZONE. UP UNTIL THIS TIME, ALL INSTRUMENTS WERE INDICATING NORMAL. WHILE IN THE
ORBIT, THE GUNNER, ^SP4 MARK J. DONAHUE,^ INFORMED THE PILOTS OF A UNUSUAL NOISE
COMING FROM THE ENGINE COMPARTMENT, AND SIMULTANEOUSLY, THE MASTER CAUTION LIGHT
CAME ON. THIS WAS IMMEDIATELY DETECTED BY ^MR.
LEE^ WHO TOLD THE PILOT TO LOOK
AT THE MASTER CAUTION PANEL TO DETERMINE THE CAUSE OF THE CAUTION LIGHT. THE
PILOT, ^WO1 VICTOR R. MASTERJOHN^, LOOKED AT THE MASTER CAUTION PANEL AND
INFORMED ^MR.
LEE^ THAT THE ENGINE CHIP DETECTOR LIGHT WAS ON. THEN THE ENGINE
RPM BEGAN DETERIORATING, AND THE AUDIO WARNING DEVICE ALONG WITH THE RPM WARNING
LIGHT CAME ON. BOTH PILOTS IMMEDIATELY RECOGNIZED AN ENGINE FAILURE AND ^MR.
LEE^ ENTERED AUTOROTATION. ^MR. MASTERJOHN^ CONTINUED TO MONITOR THE INSTRUMENTS
AND HE SAW THAT THE N1 HAD DROPPED TO APPROXIMATELY 10% THE TORQUE WAS ZERO,
ENGINE OIL PRESSURE WAS ZERO, AND THAT THE ENGINE RPM WAS BELOW 2000 AND STILL
FALLING. HE DID NOT REMEMBER THE EGT READING. ROTOR RPM REMAINED WITHIN THE
OPERATIONAL LIMITS. ^MR.
LEE^ TURNED THE AIRCRAFT TOWARDS THE EAST TO AVOID A
MOUNTAIN RANGE TO THE WEST, AND BEGAN LOOKING FOR A POSSIBLE LANDING SITE.
DURING DESCENT, HE CONTINUED TO MANUEVER THE AIRCRAFT IN SUCH A MANNER AS TO
KEEP THE ROTOR RPM WITHIN LIMITS. AT APPROXIMATELY 150 FEET ABOVE THE GROUND, A
POSSIBLE LANDING SITE WAS SPOTTED AND ^MR.
LEE^ MADE HIS APPROACH TO THIS AREA.
HE TERMINATED THE APPROACH AT A TEN FOOT HOVER TO AVOID OVERSHOOTING THE AREA.
THE AIRCRAFT THEN SETTLED STRAIGHT DOWN INTO THE LANDING SITE WHICH HAD A SLOPE
OF APPROXIMATELY 15 DEGREES. PRIOR TO TOUCHING DOWN, ^MR. MASTERJOHN^ TURNED OFF
ALL SWITCHES AND ^SP4 DONAHUE^ HAD KICKED OFF BOTH HIS MACHINE GUN AND
AMMUNITION. THE AIRCRAFT HIT THE GROUND, BOUNCED ONCE TURNING ABOUT 90 DEGREES
IN THE PROCESS, AND THEN SETTLED. DURING THIS TIME, THE TAIL BOOM WAS SEVERED
FROM THE AIRCRAFT. THE PASSENGERS EXITED THE DOWNED AIRCRAFT AND SET UP
SECURITY, AND THE CREW PULLED OUT ALL OF THE RADIOS. TWO SHIPS FROM THE SAME
UNIT LANDED ALMOST IMMEDIATELY AND LIFTED THE CREW AND PASSENGERS OUT AND TOOK
THEM TO THE ^17TH FIELD HOSPITAL^ AT AN KHE.
